Olean General Hospital's new Subacute Inpatient Rehabilitation Program is designed for patients that need specialized rehabilitative care after an acute hospital stay for medical or surgical reasons. The goal is to allow patients to transition easily to a sub-acute level of care after a hospital stay.
About Sub-Acute or "Swing Bed" Care
The notion of a "swing bed" program, otherwise known as sub-acute rehabilitation, is to provide a level of care for patients who have suffered a stroke, congestive heart failure, or are recovering from a surgery such as hip replacement.
Regardless of what the specific circumstances are, often these patients may have completed their acute hospital stay, but they may not be able to perform certain functions such as getting dressed, walk around, or tie their shoes. They may also still require nursing care. In the past, patients at Olean General Hospital who needed this level of care - known as sub-acute because it comes after the acute post-medical/surgical care - had to be transferred to another facility. This made circumstances harder on the patient, the family, and the physician. With OGH's sub-acute program, patients can "swing" from their acute care, directly into the sub-acute program without having to leave the hospital. This allows a continuity of care with the same medical records, physician, and hospital services available.
The ultimate purpose of the sub-acute program is to combine nursing and therapist care to help each patient meet goals for their recovery and ultimately return home.Olean General Hospital’s Subacute Rehab Unit offers:

What to expect
Following a thorough evaluation by the Hospital's team of nurses and therapists, coupled with the physician's input, a treatment plan will be customized to meet the patient's needs. Most patients need a combination of physical therapy, occupational therapy, speech/swallowing therapy as well as skilled nursing care. The plan will often incorporate leisure activities into the treatment plan, providing an integral link to resuming normal life back home.
