Patient Forms
Below are several forms that may be helpful to you as a patient at Olean General Hospital. The forms are provided in PDF format,
and you will need the free Adobe Acrobat Reader to view them. If you have any questions about these forms, we invite you to call
our Patient Representative at (716) 375-6348.
Advanced Directive
An advance directive tells your doctor what kind of care you would like to have if you become unable to make medical decisions (if you are in a coma, for example). If you are admitted to Olean General Hospital, our staff will probably talk to you about advance directives.
A good advance directive describes the kind of treatment you would want depending on how sick you are. For example, the directives would describe what kind of care you want if you have an illness
that you are unlikely to recover from,
or if you are permanently unconscious.
Advance directives usually tell your doctor that you don't want certain kinds of treatment.
The Advanced Directive form below is one we use here at Olean General Hospital. You are invited to click on the link to open
the PDF file, print it, fill it out, and bring it in to the hospital. If you have questions about how to fill it out,
contact our Patient Representative at (716) 375-6348.

MOLST - Medical Orders for Life Sustaining Treatment
Honoring patient preferences is a critical element in providing quality end-of-life care. To enable physicians and other health care providers to discuss and convey a patient's wishes regarding cardiopulmonary resuscitation (CPR) and life-sustaining treatment, the New York State Department of Health has approved a physician order form, the Medical Orders for Life Sustaining Treatment (MOLST), that can be used statewide by health care providers and facilities as the legal equivalent of an inpatient Do Not Resuscitate (DNR) form.
Copies of the MOLST form and the supplemental documentation forms for adults who lack capacity or meet "exceptional circumstances"
and minors are available by using the order form available from the MOLST page on Excellus BlueCross BlueShield's Web site
https://www.excellusbcbs.com
or by calling the Olean General Hospital Quality Assurance phone number 375-7469
Health Care Proxy
According to the NYS Department of Health, the Health Care Proxy form gives the person you choose as your agent the authority to make all health care decisions for you, including the decision to remove or provide life-sustaining treatment, unless you say otherwise in this form. "Health care" means any treatment, service or procedure to diagnose or treat your physical or mental condition.
We invite you to view the following PDF that contains information about the Health Care Proxy. This link from the Department of Health also contains detailed information about what a Health Care Proxy is.
